As the leader of my group, I have communicated with each adult that I will be bringing, and I/We understand that reasonable safety precautions will be taken for each child. On behalf of each adult/guardian, we agree to not hold liable the Indiana District Assemblies of God, district leadership, the local church group, or leaders responsible for any accidents. I/We, furthermore, understand photos and videos of this event will be taken and give consent that any such taken of our children may be used in publications and/or for promotional purposes. As the leader of my group it is my responsibility to make sure background checks are on file at my church to cover each adult attending.
